Geriatric Residential Outreach Service (GRO)

• SCGH / OPH Catchment
• Contact:
  • E-referral via Rehabilitation & Aged Care [GERI] / General Geriatric Medicine [GGEM]
• Indications:
  • Geriatrician Review within residential care facility
  • Increasing falls
  • Cognitive or behavioural changes
  • Medication related issues
  • Increased hospital presentations

Metropolitan Palliative Care Services (MPaCCS)

• Contact:
  • Phone 9217 1777 (8am-4pm, Mon-Fri)
• Indications:
  • Symptom assessment and management plan in patients with life limiting illness
  • Advance Care Planning
  • Seen within 72hrs of referral
